FAMILY AND FRIENDS CPR ADULT, CHILD & INFANT TRAINING AGREEMENT

Course Duration:     About 2 hours

CPR- Family & Friends CPR teaches you how to give CPR to an adult, child, or infant. Heat attack, drowning, electric shock, and other problems may cause a victim’s heart to stop pumping blood. This is called cardiac arrest. Studies show that effective CPR right away improves survival from cardiac arrest. Many cardiac arrests happen outside a hospital. Most happen at home with family members. You can make a difference, and the first step is learning CPR. You may need to use the CPR skills you learn to help a family member or a friend. AHA created this program for anyone who wants to learn CPR and does not need a course completion card in CPR.

Pre-Payment- A full payment is required to book the date and time of the class.

Equipment Charges: In case client is not able to provide TV/DVD for the class, a rental of such equipment is charged at $25.00.

Recommendation: Have students wear comfortable clothing, as we will be working with training mannequins on the floor. It is important to understand that the mannequin portion of the class may involve strenuous physical activity. Also have students bring a pen or pencil! Participants will be responsible for notifying the instructors if they are in the active stages of, or have reason to believe they have been exposed to any infectious disease.

Completion Certification – Will not be provided for this class

Re-Schedule Fee: Deposit payment is not refundable. If you have submitted a deposit for your class, and you need to postpone the class, you have up to 120 business hours (5 business days) before class date/time without additional fees. If you have a credit account with us, and you need to postpone the class, you have also up to 120 business hours (5 business days) before class date/time to do so. There shall be a $50 re-scheduling fee added to your total if you cancel the appointment less than 120 hours (5 days) prior to your scheduled appointment. If we cancel a class for any reason, we will mail you a full refund promptly.

Downgrade Changes: Can be made up to 48 hours prior to scheduled class by sending email. You will be committed to the agreement if any downgrade changes are made within 48 hours prior to the scheduled class.

Agreement: By submitting this service order you agree to the above terms. In Addition, submission of this order and payment will secure your scheduled training date and time.
